End Well brings together a multidisciplinary community that unites design, technology, health, policy and activist initiative to transform how the world thinks about caregiving, grief, illness and the end of life experience with the ultimate goal of creating a future where ending well becomes a measure of living well.

Every cure has a starting point. Like Dr. Jonas Salk when he conquered polio, Salk scientists are dedicated to innovative biological research. Old Colony Hospice

Old Colony Hospice (OCH) has been providing the best in hospice care since 1979. The organization provides quality, compassionate health care to patients facing the advanced stages of life-threatening and life-limiting illnesses. OCH provides a comprehensive range of medical and psychosocial services. OCH is dedicated to ensuring all patients and their families are at peace, treated with respect and dignity and that comfort is maintained at all times. OCH is also committed to supporting families after their loss through bereavement outreach and organized program offerings.Old Colony Hospice also offers a comprehensive Palliative Care Program designed to provide consultation services to physicians and healthcare providers to manage pain and other troubling symptoms of patients with complex medical conditions. All of the programs are offered to patients regardless of their ability to pay for care. No patient has been denied care based on financial resources.

Kennedy's Disease Association

Founded in 2000, to help find a cure or treatment for Kennedy's Disease (KD). We financially support and promote medical research to find a cure. We create a support system for those living with KD. We share information about KD for those who seek it. We improve KD awareness in the medical community. And we increase public awareness about KD and its effects on families.

Life Health and Wellness Center

Life Health and Wellness Center was created to serve the homeless men, women and families. Our providers also serve Veterans and victims of Human Trafficking. Our clinic does not discriminate against race, color, sex, religion, sexual orientation, or ability to pay. We are a group of health care providers that provide family medicine to our patients. Many of our patients do not have health care but they do have chronic illnesses that have not been treated in years. We have contracts with the local universities and provide clinical education to students for Primary Care clinical rotations. The student are taught use evidence based methods to assess, diagnosis and treat patients and also educate patients about their current condition. We are also developing ways to sign patients up with insurance so they can become compliant with medications and therapies ordered to maintain positive health outcomes.

Mission Rabies Usa

Mission Rabies is an international NGO headquartered in the UK with offices in the USA, India and Thailand. Despite being eliminated from many countries, rabies still takes a devastating toll on the world’s most marginalized and vulnerable. Existing models of rabies prevention remain inappropriate for implementation in most countries that remain rabies endemic, because they lie at the bottom of the UN HDR rankings. Since 2013, Mission Rabies has been implementing vaccination and education programs where the need is greatest, collecting evidence and sharing lessons to make global elimination a reality. The goal of Mission Rabies is to eliminate dog-mediated human deaths caused by rabies by 2030 through mega vaccination drives and educating communities at risk on the disease.

Angel's Life Foundation Inc

Our main focus is Cystic Fibrosis (CF) awareness and to raise funds through the selling of our global crafts which include handmade Soy Candles, Jewelry, Crocheting, Pottery and much, much more. Our Story: Our daughter Hannah was born with Cystic Fibrosis, a genetic life-threatening disease. Our family’s and friends mission in life has been to make a difference by raising awareness through volunteering and fundraising for Cystic Fibrosis. Our lives drastically changed on July 22nd, 2012 when our only other child, our son, SPC Brenden N. Salazar-Nelson was killed in the line of duty in Afghanistan, decorated with the Bronze Star and Purple Heart. I created Angel’s Life in 2003, I chose this name because it is the symbol of peace and salvation. Now this name means that more than ever. Together with my friends and family, we create soy-based candles and sell them to help draw awareness for Cystic Fibrosis.
