CHC'S MISSION IS TO IMPROVE THE QUALITY OF LIVING THROUGHOUT NORTHERN INDIANA, AND OUR VISION IS TO BE THE PREMIERE HOSPICE AND PALLIATIVE CARE ORGANIZATION FOR ALL END-OF-LIFE ISSUES.
