To assist our community by improving access to health and wellness, resources, and education. MCHF's goal is to build healthier communities through research and education. We will do this by assisting providers to improve the quality of care through evidenced-based process outcomes and add value to the community by providing for the individual. We seek to improve health outcomes, engagement and participation of care givers and the individual. We aim to provide tools, support and resources to empower patients to take a leadership role in their health by assisting in establishing care teams that demonstrate proactive rather than reactive patient care and improve workflow. We strive to build a cohesive system of a variety of services through programming to aid in navigating all the possibilities of an individual's health.

Our clinic does not discriminate against race, color, sex, religion, sexual orientation, or ability to pay. We are a group of health care providers that provide family medicine to our patients. Many of our patients do not have health care but they do have chronic illnesses that have not been treated in years. We have contracts with the local universities and provide clinical education to students for Primary Care clinical rotations. The student are taught use evidence based methods to assess, diagnosis and treat patients and also educate patients about their current condition. We are also developing ways to sign patients up with insurance so they can become compliant with medications and therapies ordered to maintain positive health outcomes.

Our mission is to improve the health and quality of life for all people of the greater Pajaro Valley, especially the most disenfranchised. We do this by working “upstream” (before health problems occur or worsen) to help prevent future disease through education and disease prevention, as well as “downstream” (where problems require immediate intervention or treatment) to meet the most acute need for access to quality, culturally responsive services. In all our work, we collaborate with community members and agencies and rely on data to guide our decisions. In this way, we aim to pass on stronger systems and healthier lifestyles to future generations of Pajaro Valley residents.

Since then the organization has grown and expanded to reflect the changing needs of the people and communities served. Today, the National Alliance for Hispanic Health is the premier science-based and community-driven organization that focuses on the best health for all. Community-based members provide services to more than 15 million Hispanics throughout the U.S. every year and national organization members provide services to more than 100 million people annually.
