California School-Based Health Alliance

The California School-Based Health Alliance (formerly the California School Health Centers Association) improves the health and academic success of children and youth by advancing health services in schools.  We envision a day when all of California's children and youth are healthy and achieving at their full potential. Since hiring our first paid staff in 2003, the Alliance has successfully grown to have an annual budget of more than $1,500,000 and thirteen staff.  We conduct policy work, promote the school based health center (SBHC) model, and assist SBHCs with program development to expand and strengthen school health services.  Our large network of collaborating partners includes 231 SBHCs, numerous school districts, federally qualified health centers and other providers, dozens of state and local policy organizations, and an e-communications network of more than 3,200 individuals. Our goal is to make school health centers an integral and sustainable part of the health care and educational systems. By putting health care where kids are – in schools – school health centers increase access to care and take health problems out of the classroom so that teachers can teach and students can learn.

Tanzania Health And Education Mission

The Tanzania Health and Education Mission is a California non-profit, charitable organization established in 2007 in order to support the local health clinics and schools in the Kilimanjaro region of Tanzania. Our mission is to deliver, directly to the people of Tanzania, goods and services to improve their health and education. This includes: 1 - provide medicines and vitamins for rural dispensaries inTanzania, 2 - direct payment of scholarships for student, trainees and nursery school teachers in Tanzania, and 3 - purchase and distribution of food in three rural communities in Tanzania.

My Community Health Foundation (MCHF)

We will do this by assisting providers to improve the quality of care through evidenced-based process outcomes and add value to the community by providing for the individual. We seek to improve health outcomes, engagement and participation of care givers and the individual. We aim to provide tools, support and resources to empower patients to take a leadership role in their health by assisting in establishing care teams that demonstrate proactive rather than reactive patient care and improve workflow. We strive to build a cohesive system of a variety of services through programming to aid in navigating all the possibilities of an individual's health.

Life Health and Wellness Center

Life Health and Wellness Center was created to serve the homeless men, women and families. Our providers also serve Veterans and victims of Human Trafficking. Our clinic does not discriminate against race, color, sex, religion, sexual orientation, or ability to pay. We are a group of health care providers that provide family medicine to our patients. Many of our patients do not have health care but they do have chronic illnesses that have not been treated in years. We have contracts with the local universities and provide clinical education to students for Primary Care clinical rotations. The student are taught use evidence based methods to assess, diagnosis and treat patients and also educate patients about their current condition. We are also developing ways to sign patients up with insurance so they can become compliant with medications and therapies ordered to maintain positive health outcomes.
