Fox Valley Memory Project

In collaboration with others, Fox Valley Memory Project builds communities that welcome, include, and support those with a dementia or memory loss disability as well as those who care for them.

Bridge21 Park City

Bridge21 Park City recognizes the isolation crisis individuals with disabilities face while approaching adulthood. Opportunities are growing within the Park City community for this population, however there are not enough Neurodiverse Housing options. We believe Park City and Summit County are ready to build a more inclusive community and Bridge21 seeks to create a network of bridges connecting individuals with disabilities to local housing. Ultimately, Bridge21 aims to build a Neurodiverse Co-Housing neighborhood.

Fairhaven Foundation

The Fairhaven Foundation transforms the lives of those eligible for services through the Trumbull County Board of Developmental Disabilities. They enhance the quality of life of eligible individuals by intentionally engaging them in impactful programming, and supporting meaningful outcomes.

Colten Carney Memorial Fund

Colten Carney Memorial Fund is a non-profit tax exempt organization. They provide scholarships to individuals with intellectual and developmental disabilities with funds they recieve. Along with providing companion and service dogs to those same individuals and their families.

Wings For Children And Families

Conard House

These events disrupt their ability to work, to express themselves clearly and even to eat or sleep. Negotiating the maze of healthcare bureaucracies to get the help they need can be beyond the capabilities of those who need help the most. We focus on building welcoming and caring communities that empower residents and give them hope for the future.

Oaks Integrated Care

Oaks Integrated Care is a private, nonprofit organization dedicated to improving the quality of life for vulnerable adults, children and families living with a mental illness, addiction or developmental disability. The organization offers over 150 health and social service programs throughout New Jersey designed to meet the needs of our community with compassion. By focusing on integrated care, it becomes possible to treat the whole person to achieve both mental and physical wellness. Through each of life's challenges, our role remains to help each person make healthy choices, achieve personal goals and eventually reach his or her full potential.
