Our Guiding Principles: *Equity and Access *Evidence and Outcomes-based Practice *Innovation *Collaboration *Community-oriented Our Goals & Activities In our endeavor to achieve systemic and sustainable change through education, policy and environmental strategies, we: *Offer regional trainings and technical assistance through grant funding from federal, state and private agencies *Provide site-based trainings and technical assistance on request from school districts and community organizations *Develop online programs, student learning modules and curricula *Assist with professional licensure, CEU and graduate-level credit *Create strategic partnerships with other non-profit organizations and school systems CHL is an outgrowth of an initiative by the Center for Disease Control's Division of Adolescent and School Health to build local capacity for school and community health promotion through training centers that foster and deliver high-quality professional development and practice improvement initiatives. We have substantial experience designing and delivering projects, conferences and professional development opportunities. From instruction and technical assistance to research and report writing, curricula creation and evaluation resources, CHL has served over 20,000 professionals and community members, delivered services for more than 100 projects and developed and promoted numerous local and statewide comprehensive and coordinated strategic health initiatives. CHL has the following current programs and projects: 1. Suicide Prevention- * CHL supports state-wide suicide prevention efforts through the Vermont Suicide Prevention Center, a public/private partnership which seeks to foster a sustainable approach to suicide prevention in Vermont. *CHL manages the VT Suicide Prevention Coalition consisting of representatives from public health, education, state agencies, advocacy groups, youth, mental health services, and survivors. *CHL manages, develops and oversees state-wide suicide prevention trainings including Gatekeeper Training, a Zero Suicide Practice Institute and the Collaborative Assessment for the Management of Suicidality (CAMS) trainings; *CHL coordinates the VT Gun Shop Project in collaboration with the VT Department of Mental Health. *CHL implements the Zero Suicide approach in Vermont and coordinates Zero Suicide Pilot Projects. *CHL and the VT Suicide Prevention Center coordinate the annual Vermont Suicide Prevention Symposium, a cross-agency statewide event that brings together professionals across all sectors for a day of learning from national experts on critical topics and skills in suicide prevention, treatment, and recovery. 2. Prevention Training and Workforce Development - CHL coordinates and implements Substance Abuse Disorder prevention trainings and workforce development activities for the Vermont Department of Health/Division of Alcohol and Drug Abuse Programs for Regional Prevention Partnership professional staff. 3. Annual Symposia and Conferences: In addition to the Suicide Prevention Symposium, our cornerstone event, CHL assists with the coordination of annual Symposia and Conferences for various partners including the following events: o 4th Annual College Symposium on Substance Use o Community Solutions to the Opiate Issues Facing Vermonters for the VT Department of Health o Substance Abuse Workforce Development Conference for the VT Department of Health. 4. CHL Proprietary and Fee for Service: CHL has several proprietary programs developed and implemented by the organization: *Umatter Suicide Prevention for Schools and Umatter for Staff and Family Awareness -CHL developed this best practice training for school and community settings. *Umatter for Youth and Young Adults Mental Health Wellness Promotion and Community Action--CHL is moving the focus on mental health "upstream" from intervention and crisis to promotion and prevention (self-care, coping and communication skills, etc.). *Community / School District Trainings--CHL provides professional development, technical assistance and support for the implementation of coordinated approaches to school health, and conducts training on mental health, suicide prevention and alcohol, tobacco, sexual health and other critical health topics. *Family Engagement- CHL is building an initiative to promote effective practices for Family Engagement - an emerging interest across sectors

The Network aspires to a health care system that is guided by social justice and reflects the needs of diverse women.

Mali Health empowers Sikoro, an impoverished urban community in Mali, West Africa to transform maternal and child health sustainably. We do so by: -Fostering the agency of residents and community structures to mobilize to address community health needs. -Promoting health education, prevention, and early care seeking. -Enhancing financial, geographic, and cultural access to health care for poor families.

Indigenous Health Solutions is a trans-disciplinary collective of pioneers driven by a passion for service to the Earth, and the poorest and most remote communities on it. Through the lens of planetary health, where shifts in natural systems are prioritized in examination of human health, experts in conservation, health, anthropology, and business come together with those in need to craft and implement culturally informed and community led solutions for development. Our programs are built upon the foundational principle that development must be indigenous, that is, planned in partnership with those in need and rooted in the place of delivery, reflecting practical awareness of the interconnection between health, conservation, livelihood, and education.

Physicians for Reproductive Health is a doctor-led national advocacy organization. We use evidence-based medicine to promote sound reproductive health policies. We believe in reproductive choice for everyone.

Oak rural health organization is a non governmental, not for profit organization. Registered under the CAMA 1990 as an incorporated trustee by the Corporate Affairs Commission of the Federal republic of Nigeria. OUR MISSION/AIMS AND OBJECTIVES ARE; To increase preventive health publicity and awareness among rural dwellers. To eradicate children malnutrition in the grassroots through supplementary and complementary feed assistance. To partner with rural communities on health promotion and sustenance strategies. To prevent/eradicate mortalities through mobile clinics and health outreaches in rural communities. To partner with government, private and individuals to combat pressing health challenges in the rural settlements nationwide. To establish/refurbish healthcare points in vulnerable rural communities. OUR VISION: ''To be an internationally recognized leading grassroots health advocacy gladiator, ensuring a healthy and productive rural lives''. Achieving the above, the organization engages the services of volunteer professionals in relevant fields. Also mobilize resources through donation from trustees, individuals, private and government establishments. Financial and professional integrities are strictly maintained as enshrined in the organization's articles of incorporation. Most importantly, the organization gives no room for discrimination of any nature. Irrespective of religion, tribe, race, color or ethnic background. We deal with humanity as a bio-psycho-social being. Our group of dedicated volunteers work as a team to achieve a common goal. Furthermore, the founders are not unaware of the various government and other concerned agencies' efforts on providing quality health care services to the people at the niches and crevices of the nation. Various stakeholders who invested on healthcare services often concentrate on the urbans and densely populated areas. Governments' health infrastructures in rural settlements are not enough compared to their population. Most of the rural areas are fast becoming the den of unskilled care givers, this has led to many unreported, but avoidable morbidity and mortality. Demographically, rural population is made of about 52% of the nation's total population. Distributed through the nooks and crannies. Rural dwellers cannot be underestimated in the area of country's food security and economic growth. Therefore, a healthy rural life is a healthy nation. WHY RURAL HEALTH? Rural health is a worthy advocacy because: They are the hope of nation's food security. They are the custodians of the nation's cultural heritage. They are the most vulnerable. They are the major workforce in production line. They are the future of the nation. They have the highest fertility rate. They are majorly the poor. Furthermore, the executive summary of the WHO's vision 2020 is, attainment by all people of the world, a level of health that will permit them to live a socially and economically productive life. At least, such level of health that they are capable of working productively and participating actively in the social lives of the community in which they live. Meanwhile, growing cost of quality healthcare services has put the rural lives at a disadvantageous position. WHO's vision 2020 and even beyond is however a collaborative efforts of individuals, governments and non governmental bodies.

To conduct training programmes for Community Health Workers in collaboration with other community based organizations and donor agencies. To achieve simple treatment goals through cost effective local medicines including Herbs and Folk Home Remedies designed to cure as many patients as possible with few side effects. To provide best possible treatment to the poor and needy patients through qualified and specialist doctors. To develop a Health Education Programme designed to improve the quality of life through preventative measures. To conduct training programmes for Community Health Workers in collaboration with other community based organizations and donor agencies. To establish a Centre of Excellence for the treatment of Tuberculosis (in line with WHOs, DOT programme), Hepatitis-C and other Infectious Diseases. To provide immediate relief in case of natural disasters and calamities and also to take active part in rehabilitation of the affected population.

CCI Health & Wellness Services is a group practice, empowering patients to partner with staff for an unparalleled healthcare experience. The CCI Adolescent Services provides teens and young adults (ages 12-35) in the Washington, DC Metropolitan area with affordable, timely and culturally-competent direct clinical care and educational outreach services. Direct Clinical Care- Quality Care: CCI serves as a reproductive medical home for patients ages 12-35 by providing a wide range of evidence based clinical services. Affordable: CCI works with every patient, regardless of income level or insurance status, to ensure they have access to reproductive healthcare. Comfortable: CCI provides a warm environment, where patients feel each staff member respects their individuality and is committed to their wellbeing and overall health outcomes. Accessible: Appointments are regularly available (including walk-ins) for all services; clinics are easily accessible by car, bus or metro; and all methods of contraception are available for purchase on site. Integrated Care: Medical staff, case managers and social workers collaborate to ensure continuity of care for each patient, including referrals and access to community resources. Education and Outreach -Outreach: CCI identifies members of the community at greatest risk and actively recruits them through community activities, educational presentations and partnerships with related organizations. Education: CCI provides in-clinic and community based educational interventions designed to increase knowledge, build skills and reduce risk to promote positive and responsible reproductive health behaviors.

The National Institute for Reproductive Health (NIRH) works across the country to advance proactive policy on the state and local levels to ensure that all people have access to reproductive health care, including abortion care. Using a partnership model, we provide state and local advocates with strategic guidance, hands-on support, and funding to create national change from the ground up.
