Hearts With A Mission

Hearts With A Mission serves homeless, runaway, and transitional youth in crisis by providing shelter, educational support, mentoring, family reunification, and transition planning with a faith-based approach.

A Child's Place Of Charlotte

A Child's Place works to erase the impact of homelessness on children and their education. Our vision is to provide stability within the classroom for homeless, at-risk students and, at the same time, assess and strengthen their families to remove barriers that led to the homeless crisis. Our strategy has two essential components: resolve the child's immediate basic needs while creating a case plan that supports his or her continued focus on education in one of our partner schools; and assess and advocate on behalf of the student's family through a case plan to resolve issues that led to the family's immediate homeless crisis. This unique partnership creates a holistic approach to encourage our students to stay focused on their education.

Family Preservation and Strengthening Services

Family Pass helps families that are homeless or at risk of homelessness in Fairfax county. We provide rent subsidies and connect families with the support services needed to become self-sufficient. Our work is part of fairfax county’s 10-year plan to prevent and end homelessness in our community.

Alliance To End Homelessness In Suburban Cook County

The Alliance is responsible for planning and coordinating homeless services and housing options for all of Cook County outside of Chicago.  Established in 1997 as the Task Force on Homelessness, the group changed its name to the Alliance to End Homelessness in Suburban Cook County, formally incorporated in 2004 and secured nonprofit status in 2005. The Alliance leads a diverse network of partners working together to develop, fund, and implement solutions to end homelessness.

Care Association

Care Association plans, develops, and implements innovative strategies to alleviate economic burdens on lower- and middle-income households. We serve households in the United States that pay more than 30% of their gross income on housing or are transient because of lack of affordable housing. Our projects include but are not limited to real property development, assistance for emotional resolution of blocks to achieving economic wellness, and web application development.

Wyoming Dementia Care

Our mission is to provide a better quality of life for the families and caregivers of those facing the daily challenges of dementing disease, including Alzheimer's. We provide up-to-date information about the diseases, free assistance for planning short and long-term care options and facilitate support groups with respite care. By working with other organizations in our community, we provide individual counseling sessions and art activities for the caregivers and their loved ones.

Good Seed Community Development

To help youth not only live, but thrive. We provide supportive, nurturing and specialized care for homeless young people in California through supportive housing, job training, comprehensive services and individual planning. We create an environment where youth, ages 18-25, can thrive and grow to achieve their full potential. Our aim is to ensure that youth have the tools they need to realize their goals and dreams, so that they may live healthy and meaningful lives.

Lift Orlando Inc

THEIR MISSION "LIFT Orlando exists to break the cycle of poverty through neighborhood revitalization. LIFT is a group of business leaders in the Orlando area, partnering with residents to see change in the area surrounding the Orlando Citrus Bowl. We plan to do this through our four goals of mixed income housing, health and wellness, cradle to career education pipeline, and economic development. We utilize an asset based approach, engaging and working closely with residents in our focus area."
