Founded in 1943, the mission of WJCS, a not-for-profit organization, is to provide care to people of all ages and backgrounds who are confronting significant challenges relating to mental health, trauma, disabilities, youth, and geriatric services. WJCS’ highly trained staff serve more than 20,000 Westchester County residents each year through approximately 88+ programs. Through private funding and support from UJA-Federation of New York, WJCS also meets the diverse needs of the Westchester Jewish community, providing educational and spiritual programming in Jewish day schools, nursery schools, and synagogues.

Meals on Wheels America is the leadership organization supporting the more than 5,000 community-based programs across the country that are dedicated to addressing senior isolation and hunger. This network serves virtually every community in America and, along with more than two million staff and volunteers, delivers the nutritious meals, friendly visits and safety checks that enable America’s seniors to live nourished lives with independence and dignity. By providing funding, leadership, education, research and advocacy support, Meals on Wheels America empowers its local member programs to strengthen their communities, one senior at a time. Community Bridges envisions a thriving community where every person has the opportunity to unleash their full potential. Our family of ten programs delivers essential services, provides equitable access to resources, and advocates for health and dignity across every stage of life. Every year, we meet the needs of thousands of local children, families and seniors with broad support from individuals, businesses, foundations and government. We believe that when we work together, anything is possible. Our Child Development Division's experienced, bilingual teachers offer early education opportunities, developmental screenings, and family engagement at six centers throughout the county. Child & Adult Care Food Program is a nutrition assistance program that ensures low-income children and adults attending licensed facilities receive healthy meals and each every day. Elderday Adult Day Health Care is the only program of its kind in Santa Cruz County, serving approximately 70 elders and adults with physical, emotional, or cognitive impairments each day. It provides the services this vulnerable population needs to stay healthy and maintain the highest level of independence possible. Our Family Resource Collective works with individuals and families to build safe, healthy, and compassionate communities through education, resources, and advocacy. It includes La Manzana Community Resources, Live Oak Family Resource Center, Mountain Community Resources, and Nueva Vista. Lift Line provides accessible rides to low-income, disabled, elderly and frail individuals each year, ensuring that their medical, nutritional, and emotional needs are met. Meals on Wheels for Santa Cruz County serves more than 600 meals per day to low-income, frail, elderly, and disabled residents. With no required payment or waiting list, those in need can count on a warm meal at one of our five centers, or delivered straight to their doors. Women, Infants & Children (WIC) provides nutrition education, food stamps, and breastfeeding support to more than 9,000 low-income pregnant women, infants and children each month, helping families thrive.

Butler Meals on Wheels, Inc. Mission Statement Butler Meals on Wheels, Inc. is a not-for-profit freestanding organization governed by a volunteer Board of Directors. The purpose of Butler Meals on Wheels, Inc. is to provide excellent, nutritious, home delivered meals to individuals in need within the service area comprising Butler city and it's environs. The meals are provided to maintain adequate nutrition for those recuperating from illness and suffering impaired functional ability. Butler Meals on Wheels, Inc. strives to maintain clients in their homes for as long as possible, preventing institutionalization.No resident of the Butler Meals on Wheels, Inc. service area will be denied service, or otherwise be discriminated against, based on race, color, national origin, gender, income or on any other basis. However, no person will be admitted for service unless he meets the eligibility requirements as established by the Board of Directors. Individuals referred for service will be admitted based on priority, according to needs and the adequacy and availability of personnel and resources to provide the service. Priorities for providing meals are as follows: 1. Inability to leave the home without assistance of another or without using assistive devices. 2. Discharge from an acute care facility. 3. Functional disability which significantly limits the client's ability to prepare meals for himself.4. At risk: serious consequences will result if adequate nutrition is not maintained. 5. Lack of support from family, friends, or other agencies to provide meals.

Our mission is to provide comfort care at the end of life. We believe that a life fully lived, no matter how long, should include an end-of-life experience of comfort, dignity, and peace. Our goal is to keep each patient as comfortable and independent as possible, while supporting the emotional and spiritual needs of our patients and their loved ones. We were the first non-profit Hospice in the area and one of the first 100 Hospices in the nation. Our founders were a group of dedicated doctors, nurses and community volunteers that began providing care to the dying on their own time. Our organization began in 1979 before there was insurance reimbursement for Hospice care and this dedication to our patients remains our culture to this day. Ionia Area Hospice approached Hospice of Lansing joined forces and a formal partnership was formed in 1984. Should the time come when it is no longer possible for a patient to remain at home, Stoneleigh Residence is a unique, comfortable, peaceful, and home-like alternative. Surrounded by gardens, a cascading pond, trees and wildlife, the Residence is designed to take full advantage of the natural beauty of its wooded 20-acre site. We are a true community-based Hospice, created out of necessity and driven by our mission to serve all by bringing kindness, respect and quality to the end-of-life. Hospice of Lansing, Stoneleigh Residence and Ionia Area Hospice provide pain management and personal care, respite care, emotional and spiritual support for patients, volunteer support and grief support for family members after the loss.

We coordinate and deliver comprehensive, cost effective services including nursing, physical therapy, occupational therapy, speech therapy, social work and home health aide care. In the spirit of volunteerism and community partnership, Rebuilding Together Metro Chicago improves the homes and neighborhoods of elderly, disabled and low-income residents so that they may continue to live in warmth, safety and comfort. Rebuilding Together Metro Chicago was founded to improve the declining housing stock of homeowners in Chicago and Cook County. We serve existing homeowners, who, due to financial or physical limitations, have been unable to maintain their homes. Rebuilding Together Metro Chicago was formed in 1991 as an affiliate of the national organization, Rebuilding Together. Since then, Rebuilding Together Metro Chicago has partnered with over 600 sponsors to keep homeowners living in warmth, safety and independence. To date we have repaired over 1,600 homes and 200 nonprofit facilities in partnership with communities, corporations, trade unions, and civic groups. Our work is performed by an all-volunteer labor force, enabling us to provide repair services at no cost to our clients. In addition to physical improvements, we provide personal contact, increased understanding, and an opportunity to overcome socio-economic stereotypes. We have found that our efforts often act as a catalyst for additional neighborhood improvements. How We Are Unique Meet Basic Needs & Serve the Underserved: Our programs allow clients to use their limited budget for basic essentials like food and medication, rather than costly home repairs. We do not require sweat equity and never charge for our services. Holistic Approach: We address the needs of each home and its occupants, with a focus on health and safety. Exceptional Business Model: Every $1 we receive turns into $3.25 worth of services to residents in low-income neighborhoods-- resulting in $2 million in community reinvestment every year. We offer opportunities for volunteers to make a real difference in the community & meet people from all walks of life.

Meals on Wheels of Eastern Onondaga County's mission is to provide nutritionally balanced and delicious tasting food to people unable to prepare or provide healthy meals for themselves; enabling healthy independence and facilitating aging at home. We serve the East Syracuse, Minoa, Kirkville, Bridgeport, Fayetteville, Manlius, Jamesville, DeWitt, Fabius, Pompey, LaFayette, and Tully communities within New York

Peace Village is a retirement community that houses older adults ages 65 and above in independent living, assisted living, and memory care settings. Peace Village is located in the suburban Cook County township of Palos Park, IL. Peace Village aims to create a society that re-imagines and celebrates aging by carrying out its mission of “embracing the continuing vitality of our residents by fostering a loving and respectful community in which our residents' individuality is honored, wellness is supported, faith is inspired and friendships are made."

Woodstock Area Meals on Wheels prepares and delivers hot, wholesome meals every day to the homes of the elderly, shut-ins, handicapped and convalescents within the area of Woodstock. This service is available to those who are physically unable to prepare a daily hot meal for themselves. Meals on Wheels can accommodate those with special dietary needs, including vegetarians, diabetics, and individuals with food allergies. If additional meals are needed on the weekend, they can be delivered on Friday.
