We serve people over a range of ages and disabilities from children diagnosed with developmental delays at birth to the unique needs of seniors with disabilities and all ages in between. Annually, we support over 8,000 individuals as well as their families in 15 counties, in over 160 communities and 50 locations throughout Chicago, the suburbs, and northern Illinois. We are proud to be the largest provider of home-based services in Illinois.

National Scleroderma Foundation

The National Scleroderma Foundation is a 501(c)(3) nonprofit organization founded in 1998 to advance medical research, promote disease awareness, and provide support and education to people with scleroderma, their families and support networks. Supported by a network of thousands of individuals across the United States, the Foundation helps those living with scleroderma by providing support and education at the same time that it funds peer-reviewed scleroderma research. Since its founding, the Foundation has committed over $30 Million to discover the cause, understand the mechanism, and overcome scleroderma forever. The Foundation is home to the National Scleroderma Conference—the only educational program of its kind and scope in the U.S.—which provides access to leading scleroderma experts and up-to-date information while serving as the central meeting ground for the scleroderma community. In addition, the Foundation's Stepping Out to Cure Scleroderma walks are the country's premier awareness and fundraising events which are organized by the Foundation’s local chapters and take place throughout the year at multiple locations and virtually across the country. The Foundation is led by a dedicated, volunteer Board of Directors that exercises its fiduciary responsibilities, an extraordinary, volunteer Medical & Scientific Advisory Board comprised of world-renowned physicians and scientists, and an exemplary professional staff committed to advancing its mission through the organization's cultural values of care, connection, diversity, integrity, meaningful work, and trust.

Lion Heart Heroes Foundation

The Lion Heart Heroes Foundation seeks to promote heart health for our military community. Objectives: Raise awareness of sudden cardiac arrest (SCA) and advocate for heart health in the military community. Promote the presence and use of Automated External Defibrillators (AEDS) on military posts and at physical fitness events. Support research and partner with the Navy to develop cardiac screening protocols to help identify cardiac abnormalities within our military community. Demonstrate heart screening protocols using the latest scientific techniques.

The Women's Home

The mission of The Women's Home is to build communities that strengthen women and support families as they reclaim their stability. The Women's Home has worked to build whole lives in the Houston area since our creation in 1957. Our services focus on vulnerable households with a special emphasis on women and families. We have four unique programs -- a residential treatment program located near downtown, and two permanent housing programs and a collaborative service center in Spring Branch -- that offer tailored housing, behavioral health care, and wrap around support services to help women, children, and families build resilient lives.

We were established in 1955 as a home for needy women. Through the years we have continued to offer effective programs to meet their needs. Our residents are homeless, many have substance abuse problems, some are abused and some are the abusers; some are out of income and resources. All are out of options. Evangel Home is a Christian ministry that believes in the worth of each individual and in the power of Jesus Christ to restore broken lives. We believe that each woman must learn to take responsibility for her life and our programs are aimed to help them become victors rather than to remain victims of harsh circumstances. Our basic philosophy is that we are most effective focusing on one woman 1,000 ways rather than 1,000 women one way.
