The clients we serve are deserving of a name that fits just how hard they work, how talented they are, and how much potential each person has. We call our clients "ACEs," or Adults Creating Excellence. We believe that every ACE we serve, regardless of diagnosis, disability, or circumstance, can excel with the support of their loved ones, staff, and the community. Every day, our ACEs learn new skills, foster friendships, create art, and enjoy being a part of an inclusive, caring group.

The mission of New Hope Housing is to provide homeless individuals and families shelter and the tools to build a better life.Founded in 1977, New Hope Housing is an innovative, award-winning nonprofit agency in Northern Virginia providing shelter, permanent supportive housing, outreach and prevention assistance, and support services for homeless families and single adults. New Hope Housing is committed to finding creative and lasting solutions to end the cycle of homelessness by offering homeless men, women, and children the services they need to change their lives and succeed. We believe that each individual success story contributes to a stronger, healthier community for all of us. New Hope Housing operates programs in Fairfax County, Arlington County, Alexandria, and Falls Church.

St. Mary’s General Hospital is an adult acute-care facility.As a 191-bed community hospital, we embrace progress and change while maintaining our commitment to the mission and vision of our founder’s. We offer a full range of patient services that focus on enhanced patient care and the health care needs of our community. We are home to the Regional Cardiac Care Centre and also offer emergency medicine, spiritual care, counselling, and treatment of sexual assault and domestic violence. Approximately 87% of surgeries done at St. Mary's are performed on a same-day basis using the most advanced minimally-invasive techniques. At Alzheimer's Family Services Center (AFSC), we believe that all memory-impaired seniors, irrespective of ethnic background and socioeconomic status, deserve the right to superior personalized care that will enable them to age with dignity at home. Across the last three decades, this belief has guided our mission to improve quality of life for families challenged by Alzheimer's disease or another dementia through services tailored to meet individual needs. We play a key role in our community's continuum of long-term care services by offering affordable access to dementia-specific adult day health care programs, a variety of support services to help caregivers manage the day-to-day challenges of care, and community dementia education and outreach.

The National Association of the Deaf was established in 1880 by deaf leaders who believed in the right of the American deaf community to use sign language, to congregate on issues important to them, and to have its interests represented at the national level. These beliefs remain true to this day, with American Sign Language as a core value. As a nonprofit federation, the mission of the NAD is to preserve, protect, and promote the civil, human, and linguistic rights of deaf Americans.The advocacy scope of the National Association of the Deaf is broad, covering the breadth of a lifetime and impacting future generations in the areas of early intervention, education, employment, health care, technology, telecommunications, youth development, and more.On the global front, the NAD represents the U.S.A. as an affiliate of the World Federation of the Deaf, an international human rights organization.

The Long Island Alzheimer’s & Dementia Center is dedicated to making a positive difference in the lives of those impacted by Alzheimer’s disease and related forms of dementia — one person, one family, one community at a time. We offer a full range of caregiver services along with safe, structured and engaging programs for diagnosed individuals. Our stage-specific programs are designed around an individual’s abilities, not disabilities, on what they can still do, happily and productively, which allows us to bring out the best in every participant. In addition to our day programs Monday – Saturday, we offer innovative solutions for caregivers and families. For over 30 years, the Center has believed that until there is a cure, we are the best solution to find hope and support.

Founded in 2009, The Grassroot Project (TGP) uses the power of sports and the platform of athlete role models to educate, inspire, and mobilize youth to live healthy lives. We are a team of more than 1,000 NCAA varsity athletes and 5,000 DC teens who are committed to making our city healthier. The Grassroot Project capitalizes on the excitement, relatability, and popularity of sports to provide much-needed health literacy and social empowerment programs to DC teens. The only way for us to succeed is to believe in the power of youth to make a difference. In addition to providing health education to DC teens, we invest in the leadership training, cultural competency, and professional skills of hundreds of NCAA varsity athletes who serve as our program facilitators. At the Dr. Peter AIDS Foundation, every day is an inspiring reminder of what we can achieve by providing HIV treatment and other supports for people whose lives are impacted by HIV/AIDS and the additional challenges of addiction, mental illness, poverty and discrimination. The Day Health Program and 24-hour specialized care Residence at the Dr. Peter Centre improve people's health and make a significant contribution to Vancouver's complex health care system. The Centre's progressive model of HIV/AIDS care is a source of inspiration for local, national, and international health care organizations and policy makers in responding to the new challenges in HIV/AIDS care. Our community of care is built on acceptance and support, helping people find strength in the face of an illness which does not have a cure. Our mission is to deliver a breadth of resources to caregivers, families and patients who are confronting the final stages of life. Vision & Values Statement SCHF exists to elevate the quality of life experienced by those confronting a terminal illness. Our services support not just the patient, but family members and caregivers as well. As a result, we are continuously working with the care team to fulfill our mission, all the while, never losing sight of our vision: to open a Hospice House in Southern California that serves as a live-in facility for end-of-life care. We Believe: Every life is precious… Every life deserves to be celebrated… Each moment should be lived to its fullest… These are the fundamental values that drive the Southern California Hospice Foundation to continually reach out and serve those struggling with the complex issues surrounding end-of-life.

The Hospital for Sick Children Foundation, affectionately known as SickKids Foundation, is the largest non-governmental granting agency in child health in Canada. Established in 1972, SickKids Foundation has granted over $300 million to The Hospital for Sick Children and over $60 million to researchers across the country. Through its National Grants Program, SickKids Foundation is the only granting program with a national, multi-year initiative to study two understudied but important areas of child health: the role of complementary and alternative health care and child health, and to study home care and children and youth. We have invested our community’s contributions in outstanding patient care, research and education. We believe there is no one else in Canada as dedicated as we are to eliminating the gap between what is happening in child health research, education and training, and what should be happening.
