Afrika Tikkun was founded in 1994 soon after the emergence of Democracy in South Africa. Our visionary founders-- the late Chief Rabbi Cyril Harris and philanthropist and businessman, late Bertie Lubner-- had a vision of focusing on children knowing they represented the future of the country, which inspired the-then President Nelson Mandela to volunteer as our Patron-in-Chief-- an honor that he only bestowed on four organizations during his lifetime. In 2002 he wrote: “Afrika Tikkun demonstrates in a practical and sustainable manner what can be done with limited resources, great commitment and passion. It is my belief that Afrika Tikkun represents the best of what civil society can offer in partnership with Government’s considerable efforts.” For the last 22 years, Afrika Tikkun has worked to redress the consequences of institutionalized oppression in South Africa. Our unique Cradle-to-Career development model enables our 550+ employees to deliver services addressing holistic educational, medical and economic needs of marginalized youth from early infancy through adulthood, inspiring and enabling them to become productive citizens and leaders. A Mission of ServiceThe Holyoke Visiting Nurse Association, Inc. established in 1905, is dedicated to providing quality home health care to individuals who are elderly, impoverished and medically needy. We coordinate and deliver comprehensive, cost effective services including nursing, physical therapy, occupational therapy, speech therapy, social work and home health aide care. In addition, we are committed to promoting community health through education and special programs.Our philosophy is grounded in the following values:Deep respect for the patient and family. In the spirit of volunteerism and community partnership, Rebuilding Together Metro Chicago improves the homes and neighborhoods of elderly, disabled and low-income residents so that they may continue to live in warmth, safety and comfort. Rebuilding Together Metro Chicago was founded to improve the declining housing stock of homeowners in Chicago and Cook County. We serve existing homeowners, who, due to financial or physical limitations, have been unable to maintain their homes. Rebuilding Together Metro Chicago was formed in 1991 as an affiliate of the national organization, Rebuilding Together. Since then, Rebuilding Together Metro Chicago has partnered with over 600 sponsors to keep homeowners living in warmth, safety and independence. To date we have repaired over 1,600 homes and 200 nonprofit facilities in partnership with communities, corporations, trade unions, and civic groups. Our work is performed by an all-volunteer labor force, enabling us to provide repair services at no cost to our clients. In addition to physical improvements, we provide personal contact, increased understanding, and an opportunity to overcome socio-economic stereotypes. We have found that our efforts often act as a catalyst for additional neighborhood improvements. How We Are Unique Meet Basic Needs & Serve the Underserved: Our programs allow clients to use their limited budget for basic essentials like food and medication, rather than costly home repairs. We do not require sweat equity and never charge for our services. Holistic Approach: We address the needs of each home and its occupants, with a focus on health and safety. Exceptional Business Model: Every $1 we receive turns into $3.25 worth of services to residents in low-income neighborhoods-- resulting in $2 million in community reinvestment every year. We offer opportunities for volunteers to make a real difference in the community & meet people from all walks of life.

Community Bridges envisions a thriving community where every person has the opportunity to unleash their full potential. Our family of ten programs delivers essential services, provides equitable access to resources, and advocates for health and dignity across every stage of life. Every year, we meet the needs of thousands of local children, families and seniors with broad support from individuals, businesses, foundations and government. We believe that when we work together, anything is possible. Our Child Development Division's experienced, bilingual teachers offer early education opportunities, developmental screenings, and family engagement at six centers throughout the county. Child & Adult Care Food Program is a nutrition assistance program that ensures low-income children and adults attending licensed facilities receive healthy meals and each every day. Elderday Adult Day Health Care is the only program of its kind in Santa Cruz County, serving approximately 70 elders and adults with physical, emotional, or cognitive impairments each day. It provides the services this vulnerable population needs to stay healthy and maintain the highest level of independence possible. Our Family Resource Collective works with individuals and families to build safe, healthy, and compassionate communities through education, resources, and advocacy. It includes La Manzana Community Resources, Live Oak Family Resource Center, Mountain Community Resources, and Nueva Vista. Lift Line provides accessible rides to low-income, disabled, elderly and frail individuals each year, ensuring that their medical, nutritional, and emotional needs are met. Meals on Wheels for Santa Cruz County serves more than 600 meals per day to low-income, frail, elderly, and disabled residents. With no required payment or waiting list, those in need can count on a warm meal at one of our five centers, or delivered straight to their doors. Women, Infants & Children (WIC) provides nutrition education, food stamps, and breastfeeding support to more than 9,000 low-income pregnant women, infants and children each month, helping families thrive.

The Little Sisters of the Poor are an international congregation of Roman Catholic women religious founded in 1839 by Saint Jeanne Jugan. Together with a diverse network of collaborators, we serve the elderly poor in over 30 countries around the world. Continuing the work of Saint Jeanne Jugan, our MISSION is to offer the neediest elderly of every race and religion a home where they will be welcomed as Christ, cared for as family and accompanied with dignity until God calls them to himself.

The mission of Naperville Seniors in Action d/b/a Ride Assist Naperville (RAN) is to improve the quality of life for Naperville seniors and their caregivers by providing safe, affordable and dependable transportation to medical appointments. We will:• Respect the dignity of our senior passengers;• Provide a means for the employed family member to remain at work;• Strengthen the peace of mind of the senior passenger and their family and friends;• Encourage community response to help meet these needs.

Chiara Communities Foundation, Inc., a Wisconsin non-stock corporation (the Organization), is a newly-formed entity devoted exclusively to a program of support for charitable activities within the meaning of Section 501(c)(3) of the Internal Revenue Code of 1986, as amended (the Code). The Organization may also provide housing for the elderly and affordable housing for low-income individuals. The Organization is a Type II supporting organization of the Supported Organizations. The Organization will assist the Supported Organizations in meeting their charitable purposes. The Supported Organizations are nonprofit organizations that provide specially designed housing to elderly persons and low income elderly persons at the lowest feasible cost and maintains in residence those tenants who subsequently become unable to pay its monthly fees, as well as housing for low income individuals.

The mission of Meals-on-Wheels of White Plains is to deliver both food and compassion to our homebound neighbors in need. Meals-on-Wheels serves the shut-ins of our city who are unable to shop or prepare adequate meals themselves. These people are homebound due to some medical problem or condition such as a physical injury, physical illness, mental illness, recovery from surgery, etc. Meals-on-Wheels delivers two delicious and nutritionally balanced meals per day. Our goal is to bring to each client a smile and meals that taste great, look pleasing and meet their special health needs. We deliver two delicious and nutritionally balanced meals per day.
