We, the Carmelite Sisters for the Aged and Infirm, as women of the church inspired by Mother M. Angeline Teresa, respond with faith, courage and love to the healing ministry of Christ. Our philosophy is stated in her words:Our mission is reflected in the philosophy of care inherent in all policies and practices promulgated within the health care facilities under our supervision, thereby charging administration, boards of directors and medical staff with promoting the Christian mission of healing and care.On a local level, each Home has a Mission Effectiveness Coordinator. Each coordinator assists in implementing the Mission Statement and the philosophy of the Congregation throughout that particular Carmelite facility and working with other Coordinators to reinforce our mission through

Shepherd's Center of Chesterfield enriches the lives of adults 60 and over by offering opportunities for interfaith community service, lifelong learning and assistance in maintaining a healthy and independent lifestyle. We work closely with our volunteers, donors and community partners to fulfill our mission by providing: Free, accompanied transportation to medical appointments, pharmacies and for grocery shopping, including food pantriesA minor home repair service for home safety and independent living securityA low-cost lifelong learning program to help maintain overall physical and mental health and keep seniors socially connected with the community at largeA variety of opportunities to volunteer within the organization to help them stay active and to live a life that truly matters

The Little Sisters of the Poor offers life long security and care for the elderly in a home-like setting, surrounding them with thoughtful attention, spiritual support and appropriate medical care. The Little Sisters uphold the dignity and value of each life, promote the well-being of every needy elderly person and keep vigil with them when dying. The Sisters embrace poverty and depend on the generosity of others who are willing to share in their mission. The congregation was founded in Brittany, France by St. Jeanne Jugan in 1839. Since their inception, the Little Sisters have given quality care to over 1,000,000 aged poor. Today 2,900 Little Sisters serve in over 200 homes in 31 countries.

Samaritan Hospice provides specialized medical care and emotional support for people facing serious and progressive illness in the comfort of their homes or other residences. Samaritan offers an interdisciplinary team of nurses, social workers, home health aides, spiritual support coordinators, bereavement counselors and volunteers to help patients and their families live as fully and comfortably as possible through individualized plans of symptom management and emotional support. Services are provided regardless of financial circumstances. Samaritan has been an innovative provider of hospice care since its inception in 1980. It is now one of the 15 largest providers of hospice care in the country. It has cared for more patients than any other hospice in the Middle Atlantic States and remains on the cutting edge of hospice care.

Drive a Senior Southwest enriches lives by providing volunteer-based transportation, friendship, and other services at no cost to non-driving seniors helping them to maintain an independent lifestyle. Healthy Aging In Place. That’s what Drive a Senior Southwest is all about. The greater Austin area has one of the fastest growing populations of adults over 65 in the U.S. and many of those older adults no longer drive. With your help, in 2017 Drive a Senior Southwest will help over 200 seniors in southwest Austin and Travis County to continue living independently by providing over 5,000 free rides and help with minor home repairs and yard work. We drive seniors to their medical and wellness appointments and, just as important, to other commitments and activities that are vital for their well-being.

India Home Inc. is a not-for-profit organization in New York started in 2007 by healthcare providers who personally and professionally witnessed the social, psychological and medical problems of seniors. India Home is committed to providing services for seniors in culturally sensitive environment by collaborating with other not for profit organizations, individuals, health care professionals, and government organizations. Our goal is to provide quality care by adding life to the years in a constantly collaboratively environment. After seven years of providing senior center services in a culturally and linguistically appropriate environment once a week in different parts of Queens through collaborations, we opened the Desi Senior Center one year ago in Jamaica providing three days a week senior center services. As a result of this expansion, we currently serve over 150 individual seniors every week and provided over 20,000 congregate meals to South Asian seniors in just the past one year!

Afrika Tikkun was founded in 1994 soon after the emergence of Democracy in South Africa. Our visionary founders-- the late Chief Rabbi Cyril Harris and philanthropist and businessman, late Bertie Lubner-- had a vision of focusing on children knowing they represented the future of the country, which inspired the-then President Nelson Mandela to volunteer as our Patron-in-Chief-- an honor that he only bestowed on four organizations during his lifetime. In 2002 he wrote: “Afrika Tikkun demonstrates in a practical and sustainable manner what can be done with limited resources, great commitment and passion. It is my belief that Afrika Tikkun represents the best of what civil society can offer in partnership with Government’s considerable efforts.” For the last 22 years, Afrika Tikkun has worked to redress the consequences of institutionalized oppression in South Africa. Our unique Cradle-to-Career development model enables our 550+ employees to deliver services addressing holistic educational, medical and economic needs of marginalized youth from early infancy through adulthood, inspiring and enabling them to become productive citizens and leaders. We coordinate and deliver comprehensive, cost effective services including nursing, physical therapy, occupational therapy, speech therapy, social work and home health aide care. In the spirit of volunteerism and community partnership, Rebuilding Together Metro Chicago improves the homes and neighborhoods of elderly, disabled and low-income residents so that they may continue to live in warmth, safety and comfort. Rebuilding Together Metro Chicago was founded to improve the declining housing stock of homeowners in Chicago and Cook County. We serve existing homeowners, who, due to financial or physical limitations, have been unable to maintain their homes. Rebuilding Together Metro Chicago was formed in 1991 as an affiliate of the national organization, Rebuilding Together. Since then, Rebuilding Together Metro Chicago has partnered with over 600 sponsors to keep homeowners living in warmth, safety and independence. To date we have repaired over 1,600 homes and 200 nonprofit facilities in partnership with communities, corporations, trade unions, and civic groups. Our work is performed by an all-volunteer labor force, enabling us to provide repair services at no cost to our clients. In addition to physical improvements, we provide personal contact, increased understanding, and an opportunity to overcome socio-economic stereotypes. We have found that our efforts often act as a catalyst for additional neighborhood improvements. How We Are Unique Meet Basic Needs & Serve the Underserved: Our programs allow clients to use their limited budget for basic essentials like food and medication, rather than costly home repairs. We do not require sweat equity and never charge for our services. Holistic Approach: We address the needs of each home and its occupants, with a focus on health and safety. Exceptional Business Model: Every $1 we receive turns into $3.25 worth of services to residents in low-income neighborhoods-- resulting in $2 million in community reinvestment every year. We offer opportunities for volunteers to make a real difference in the community & meet people from all walks of life.

Community Bridges envisions a thriving community where every person has the opportunity to unleash their full potential. Our family of ten programs delivers essential services, provides equitable access to resources, and advocates for health and dignity across every stage of life. Every year, we meet the needs of thousands of local children, families and seniors with broad support from individuals, businesses, foundations and government. We believe that when we work together, anything is possible. Our Child Development Division's experienced, bilingual teachers offer early education opportunities, developmental screenings, and family engagement at six centers throughout the county. Child & Adult Care Food Program is a nutrition assistance program that ensures low-income children and adults attending licensed facilities receive healthy meals and each every day. Elderday Adult Day Health Care is the only program of its kind in Santa Cruz County, serving approximately 70 elders and adults with physical, emotional, or cognitive impairments each day. It provides the services this vulnerable population needs to stay healthy and maintain the highest level of independence possible. Our Family Resource Collective works with individuals and families to build safe, healthy, and compassionate communities through education, resources, and advocacy. It includes La Manzana Community Resources, Live Oak Family Resource Center, Mountain Community Resources, and Nueva Vista. Lift Line provides accessible rides to low-income, disabled, elderly and frail individuals each year, ensuring that their medical, nutritional, and emotional needs are met. Meals on Wheels for Santa Cruz County serves more than 600 meals per day to low-income, frail, elderly, and disabled residents. With no required payment or waiting list, those in need can count on a warm meal at one of our five centers, or delivered straight to their doors. Women, Infants & Children (WIC) provides nutrition education, food stamps, and breastfeeding support to more than 9,000 low-income pregnant women, infants and children each month, helping families thrive.
